Pragmatic Free Trial Meta<br /><br />Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that supports research on pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2, allowing for multiple and diverse meta-epidemiological studies to evaluate the effect of treatment on trials with different levels of pragmatism as well as other design features.<br /><br />Background<br /><br />Pragmatic trials are increasingly acknowledged as providing evidence from the real world to support clinical decision-making. However, the use of the term "pragmatic" is not uniform and its definition and evaluation requires clarification. Pragmatic trials are intended to guide clinical practices and policy decisions rather than verify a physiological hypothesis or clinical hypothesis. A pragmatic study should try to be as similar to the real-world clinical environment as is possible, including the recruitment of participants, setting and design of the intervention, its delivery and implementation of the intervention, determination and analysis of outcomes and primary analysis. This is a major difference from explanatory trials (as described by Schwartz and Lellouch1) that are designed to provide more thorough proof of an idea.<br /><br />Trials that are truly practical should be careful not to blind patients or healthcare professionals, as this may lead to bias in the estimation of the effects of treatment. The trials that are pragmatic should also try to enroll patients from a wide range of health care settings, to ensure that the results can be compared to the real world.<br /><br />Furthermore, trials that are pragmatic must concentrate on outcomes that are important to patients, like quality of life and functional recovery. This is particularly relevant in trials that require the use of invasive procedures or could have serious adverse consequences. The CRASH trial29 compared a 2-page report with an electronic monitoring system for hospitalized patients with chronic cardiac failure. The catheter trial28, however, used symptomatic catheter associated urinary tract infection as its primary outcome.<br /><br />In addition to these characteristics pragmatic trials should reduce the trial procedures and data collection requirements to reduce costs. Finaly the aim of pragmatic trials is to make their findings as applicable to current clinical practices as they can. This can be accomplished by ensuring that their primary analysis is based on the intention-to treat approach (as defined in CONSORT extensions).<br /><br />Many RCTs that don't meet the criteria for pragmatism but have features that are in opposition to pragmatism, have been published in journals of various types and incorrectly labeled pragmatic. This could lead to misleading claims of pragmatism, and the use of the term needs to be standardized. The creation of the PRECIS-2 tool, which offers an objective and standard assessment of practical features is a great first step.<br /><br />Methods<br /><br />In a practical trial, the aim is to inform policy or clinical decisions by demonstrating how the intervention can be incorporated into real-world routine care. This differs from explanation trials that test hypotheses regarding the cause-effect connection in idealized situations. In this way, pragmatic trials may have less internal validity than explanatory studies and are more susceptible to biases in their design, analysis, and conduct. In this study the areas of recruitment, organisation, flexibility in delivery, flexible adherence, and follow-up received high scores. However, the primary outcome and method of missing data was scored below the pragmatic limit. This suggests that it is possible to design a trial using good pragmatic features without damaging the quality of its outcomes.<br /><br />It is hard to determine the degree of pragmatism within a specific trial since pragmatism doesn't possess a specific attribute. Certain aspects of a research study can be more pragmatic than other. Furthermore, logistical or protocol changes during the trial may alter its score in pragmatism. But pragmatic trials can have their disadvantages. Schwartz and Lellouch1 developed a framework to discern between explanation-based studies that prove the physiological hypothesis or clinical hypothesis and pragmatic studies that guide the choice for appropriate therapies in clinical practice. The framework was composed of nine domains that were evaluated on a scale of 1-5 with 1 being more informative and 5 being more pragmatic. The domains included recruitment of intervention, setting up, delivery of intervention, flexible compliance and primary analysis.<br /><br />The original PRECIS tool3 included similar domains and scales from 1 to 5. Koppenaal and colleagues10 developed an adaptation of this assessment called the Pragmascope that was easier to use in systematic reviews. They discovered that pragmatic systematic reviews had higher average scores in the majority of domains, with lower scores in the primary analysis domain.<br /><br /><br /><br />This difference in primary analysis domains could be explained by the way most pragmatic trials analyze data. Some explanatory trials, however, do not. The overall score for pragmatic systematic reviews was lower when the domains of organization, flexible delivery, and following-up were combined.<br /><br />It is important to understand that the term "pragmatic trial" does not necessarily mean a poor quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but it is neither specific nor sensitive) that employ the term "pragmatic" in their abstracts or titles. The use of these terms in abstracts and titles could indicate a greater understanding of the importance of pragmatism but it is unclear whether this is evident in the content of the articles.<br /><br />Conclusions<br /><br />As the importance of real-world evidence becomes increasingly commonplace the pragmatic trial has gained traction in research. They are clinical trials randomized which compare real-world treatment options instead of experimental treatments in development, they involve patient populations which are more closely resembling the ones who are treated in routine care, they use comparators that are used in routine practice (e.g., existing drugs) and depend on the self-reporting of participants about outcomes. This approach can overcome the limitations of observational research, like the biases that are associated with the reliance on volunteers, as well as the insufficient availability and the coding differences in national registry.<br /><br />Other benefits of pragmatic trials include the possibility of using existing data sources, and a greater likelihood of detecting meaningful changes than traditional trials. However, they may have some limitations that limit their validity and generalizability. For example the participation rates in certain trials could be lower than anticipated due to the healthy-volunteer influence and incentives to pay or compete for participants from other research studies (e.g. industry trials). A lot of pragmatic trials are limited by the need to recruit participants quickly. Some pragmatic trials also lack controls to ensure that observed differences aren't due to biases during the trial.<br /><br />The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described as pragmatism. They evaluated pragmatism using the PRECIS-2 tool that includes the domains eligibility criteria and recruitment criteria, as well as flexibility in intervention adherence, and follow-up.
