Pragmatic Free Trial Meta<br /><br />Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that evaluate the effects of treatment across trials of different levels of pragmatism.<br /><br />Background<br /><br />Pragmatic studies provide real-world evidence that can be used to make clinical decisions. The term "pragmatic" however, is a word that is often used in contradiction and its definition and measurement need further clarification. Pragmatic trials should be designed to guide clinical practice and policy decisions, not to confirm the validity of a clinical or physiological hypothesis. A pragmatic study should aim to be as similar to real-world clinical practice as possible, such as its selection of participants, setting and design as well as the execution of the intervention, determination and analysis of outcomes as well as primary analysis. This is a major distinction between explanatory trials, as described by Schwartz and Lellouch1 which are designed to confirm the hypothesis in a more thorough way.<br /><br />Trials that are truly pragmatic should avoid attempting to blind participants or the clinicians in order to result in bias in the estimation of the effect of treatment. Similarly, the catheter trial28 used symptomatic catheter-associated urinary tract infections as the primary outcome.<br /><br />In addition to these features, pragmatic trials should minimize the procedures for conducting trials and data collection requirements to reduce costs. Finally pragmatic trials should strive to make their findings as applicable to clinical practice as possible by making sure that their primary method of analysis is based on the intention-to-treat method (as described in CONSORT extensions for pragmatic trials).<br /><br />Many RCTs that do not meet the criteria for pragmatism, however, they have characteristics that are contrary to pragmatism, have been published in journals of varying types and incorrectly labeled as pragmatic. This can lead to misleading claims about pragmatism, and the usage of the term should be standardised. The development of the PRECIS-2 tool, which offers an objective and standard assessment of pragmatic characteristics, is a good first step.<br /><br />Methods<br /><br />In a practical study, the goal is to inform policy or clinical decisions by showing how an intervention can be integrated into routine care in real-world settings. This is different from explanatory trials, which test hypotheses about the cause-effect connection in idealized settings. In this way, pragmatic trials may have a lower internal validity than studies that explain and be more prone to biases in their design analysis, conduct, and design. Despite these limitations, pragmatic trials can contribute valuable information to decisions in the context of healthcare.<br /><br />The PRECIS-2 tool measures the degree of pragmatism within an RCT by assessing it on 9 domains that range from 1 (very explanatory) to 5 (very pragmatic). In this study the domains of recruitment, organisation and flexibility in delivery, flexibility in adherence, and follow-up were awarded high scores. However, the primary outcome and method of missing data was scored below the pragmatic limit. This suggests that it is possible to design a trial with good pragmatic features without compromising the quality of its outcomes.<br /><br />It is difficult to determine the amount of pragmatism within a specific trial since pragmatism doesn't possess a specific attribute. Certain aspects of a research study can be more pragmatic than others. Furthermore, logistical or protocol modifications made during an experiment can alter its score on pragmatism. In addition 36% of 89 pragmatic trials discovered by Koppenaal and co. were placebo-controlled or conducted before approval and a majority of them were single-center. This was the case in the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not corrected for covariates that differed at baseline.<br /><br />In addition, pragmatic studies may pose challenges to collection and interpretation safety data. This is because adverse events are typically reported by participants themselves and are prone to reporting delays, inaccuracies, or coding variations. It is important to improve the accuracy and quality of outcomes in these trials.<br /><br />Results<br /><br />While the definition of pragmatism does not require that all trials be 100 100% pragmatic, there are advantages of including pragmatic elements in clinical trials. These include:<br /><br />Incorporating routine patients, the results of trials can be more quickly translated into clinical practice. However, pragmatic studies can also have disadvantages. For instance, the right type of heterogeneity can help a study to generalize its findings to a variety of patients and settings; however the wrong type of heterogeneity may reduce the assay's sensitivity, and thus decrease the ability of a study to detect minor treatment effects.<br /><br />Several studies have attempted to categorize pragmatic trials using various definitions and scoring methods. Schwartz and Lellouch1 developed a framework to distinguish between research studies that prove the clinical or physiological hypothesis as well as pragmatic trials that inform the choice of appropriate therapies in the real-world clinical setting. Their framework included nine domains that were scored on a scale ranging from 1 to 5, with 1 indicating more lucid and 5 indicating more pragmatic. The domains included recruitment, setting up, delivery of intervention, flex adherence and primary analysis.<br /><br />The initial PRECIS tool3 had similar domains and a scale of 1 to 5. Koppenaal et al10 devised an adaptation to this assessment, dubbed the Pragmascope that was simpler to use in systematic reviews. They found that pragmatic systematic reviews had higher average scores across all domains but lower scores in the primary analysis domain.<br /><br />This difference in the main analysis domain could be due to the fact that most pragmatic trials analyze their data in an intention to treat method, whereas some explanatory trials do not. The overall score for pragmatic systematic reviews was lower when the areas of management, flexible delivery and following-up were combined.<br /><br />It is important to understand that the term "pragmatic trial" does not necessarily mean a low quality trial, and in fact there is a growing number of clinical trials (as defined by MEDLINE search, but this is not sensitive nor specific) which use the word 'pragmatic' in their title or abstract. These terms may indicate a greater understanding of pragmatism in titles and abstracts, but it's unclear if this is reflected in content.<br /><br />Conclusions<br /><br />As the importance of evidence from the real world becomes more commonplace and pragmatic trials have gained momentum in research. They are randomized clinical trials which compare real-world treatment options instead of experimental treatments under development. They include patients that are more similar to the ones who are treated in routine care, they use comparisons that are commonplace in practice (e.g., existing medications), and they depend on participants' self-reports of outcomes. This approach could help overcome limitations of observational studies, such as the biases associated with reliance on volunteers, and the limited accessibility and coding flexibility in national registry systems.<br /><br /><br /><br />Other advantages of pragmatic trials include the ability to use existing data sources, and a higher chance of detecting meaningful changes than traditional trials. However, they may still have limitations which undermine their validity and generalizability. The participation rates in certain trials could be lower than anticipated due to the health-promoting effect, financial incentives, or competition from other research studies. The requirement to recruit participants in a timely fashion also reduces the size of the sample and impact of many pragmatic trials. Practical trials aren't always equipped with controls to ensure that observed differences aren't due to biases that occur during the trial.<br /><br />The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatic. The PRECIS-2 tool was used to assess the degree of pragmatism. It includes areas like eligibility criteria, recruitment flexibility, adherence to intervention, and follow-up. They discovered that 14 of the trials scored pragmatic or highly practical (i.e., scoring 5 or more) in one or more of these domains, and that the majority were single-center.<br /><br />Trials that have a high pragmatism score tend to have more expansive eligibility criteria than traditional RCTs which have very specific criteria that are not likely to be found in the clinical environment, and they contain patients from a broad variety of hospitals. The authors claim that these traits can make pragmatic trials more meaningful and useful for everyday practice, but they don't necessarily mean that a pragmatic trial is free of bias.
