Pragmatic Free Trial Meta<br /><br />Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It collects and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses that compare treatment effect estimates across trials of different levels of pragmatism.<br /><br /><br /><br />Background<br /><br />Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the use of the term "pragmatic" is inconsistent and its definition as well as assessment requires clarification. The purpose of pragmatic trials is to inform clinical practice and policy decisions, rather than to prove the validity of a clinical or physiological hypothesis. A pragmatic study should strive to be as close as is possible to actual clinical practices, including recruitment of participants, setting, design, delivery and implementation of interventions, determining and analysis results, as well as primary analyses. This is a significant difference between explanation-based trials, as defined by Schwartz &amp; Lellouch1 that are designed to confirm the hypothesis in a more thorough manner.<br /><br />The trials that are truly pragmatic should be careful not to blind patients or clinicians, as this may lead to bias in the estimation of the effects of treatment. The trials that are pragmatic should also try to attract patients from a variety of health care settings, to ensure that their findings can be applied to the real world.<br /><br />Additionally, clinical trials should focus on outcomes that matter to patients, such as the quality of life and functional recovery. This is particularly relevant for trials involving the use of invasive procedures or potentially dangerous adverse events. The CRASH trial29, for instance, focused on functional outcomes to evaluate a two-page case report with an electronic system for monitoring of patients admitted to hospitals with chronic heart failure. Similarly, the catheter trial28 focused on urinary tract infections caused by catheters as the primary outcome.<br /><br />In addition to these characteristics pragmatic trials should reduce the requirements for data collection and trial procedures to reduce costs and time commitments. In the end the aim of pragmatic trials is to make their findings as relevant to real-world clinical practice as is possible. This can be accomplished by ensuring their primary analysis is based on the intention-to treat method (as described in CONSORT extensions).<br /><br />Many RCTs which do not meet the criteria for pragmatism but contain features in opposition to pragmatism, have been published in journals of varying types and incorrectly labeled pragmatic. This can lead to false claims of pragmatism, and the usage of the term should be standardized. The development of a PRECIS-2 tool that can provide an objective, standardized evaluation of the pragmatic characteristics is a good start.<br /><br />Methods<br /><br />In a practical trial it is the intention to inform clinical or policy decisions by showing how an intervention could be integrated into everyday routine care. Explanatory trials test hypotheses about the causal-effect relationship in idealized conditions. Consequently, pragmatic trials may be less reliable than explanatory trials, and could be more susceptible to bias in their design, conduct, and analysis. Despite these limitations, pragmatic trials may be a valuable source of information for decision-making in the context of healthcare.<br /><br />The PRECIS-2 tool assesses the level of pragmatism that is present in an RCT by scoring it across 9 domains ranging from 1 (very explicative) to 5 (very pragmatic). In this study, the recruit-ment organization, flexibility in delivery, flexible adherence and follow-up domains were awarded high scores, however the primary outcome and the method for missing data were below the practical limit. This suggests that it is possible to design a trial that has high-quality pragmatic features, without compromising the quality of its outcomes.<br /><br />However, it is difficult to assess how pragmatic a particular trial really is because pragmatism is not a binary attribute; some aspects of a trial can be more pragmatic than others. Additionally, logistical or protocol modifications made during an experiment can alter its score on pragmatism. Schwartz and Lellouch1 have developed a framework that can differentiate between explanation studies that prove the physiological hypothesis or clinical hypothesis, and pragmatic studies that guide the selection of appropriate therapies in the real-world clinical practice. The framework was comprised of nine domains scored on a 1-5 scale which indicated that 1 was more explanatory while 5 was more practical. The domains covered recruitment, setting up, delivery of intervention, flex compliance and primary analysis.<br /><br />The original PRECIS tool3 included similar domains and a scale of 1 to 5. Koppenaal et al10 developed an adaptation of this assessment, dubbed the Pragmascope which was more user-friendly to use for systematic reviews. They found that pragmatic systematic reviews had higher average scores in the majority of domains but lower scores in the primary analysis domain.<br /><br />This distinction in the primary analysis domains could be due to the way in which most pragmatic trials approach data. Certain explanatory trials however, do not. The overall score for pragmatic systematic reviews was lower when the domains of organisation, flexible delivery and follow-up were merged.<br /><br />It is important to note that a pragmatic trial doesn't necessarily mean a low quality trial, and there is an increasing number of clinical trials (as defined by MEDLINE search, however it is neither specific nor sensitive) that employ the term "pragmatic" in their title or abstract. The use of these terms in titles and abstracts could suggest a greater awareness of the importance of pragmatism, but it is unclear whether this is reflected in the content of the articles.<br /><br />Conclusions<br /><br />As appreciation for the value of evidence from the real world becomes more widespread the pragmatic trial has gained momentum in research. They are randomized trials that compare real world alternatives to experimental treatments in development. They include patient populations that are more similar to those who receive treatment in regular medical care. This method is able to overcome the limitations of observational research, such as the biases associated with the reliance on volunteers and the lack of codes that vary in national registers.<br /><br />Other advantages of pragmatic trials are the ability to use existing data sources, as well as a higher probability of detecting significant changes than traditional trials. However, pragmatic tests may be prone to limitations that undermine their reliability and generalizability. The participation rates in certain trials could be lower than expected due to the health-promoting effect, financial incentives, or competition from other research studies. Many pragmatic trials are also restricted by the need to recruit participants on time.
